Tirzepatide in India: Research Grade vs Prescription, Dosing & Cost Comparison

Mounjaro (tirzepatide) costs ₹20,000+ per pen in India and has limited availability. Research-grade tirzepatide provides the same active compound at 80% lower cost. Here's everything.

Tirzepatide (branded as Mounjaro for diabetes, Zepbound for obesity) is the most successful pharmaceutical product launch in history by revenue, generating over $5 billion in its first full year. In India, it remains largely inaccessible due to cost and supply constraints. Research-grade tirzepatide is changing that equation.

What Is Tirzepatide?

Tirzepatide is a dual GIP/GLP-1 receptor agonist — a once-weekly injectable peptide that binds both the glucose-dependent insulinotropic polypeptide (GIP) receptor and the glucagon-like peptide-1 (GLP-1) receptor. This dual mechanism produces superior weight loss versus GLP-1 alone (semaglutide) because GIP and GLP-1 work synergistically for appetite suppression.

Clinical Trial Results (SURMOUNT-1)

  • Tirzepatide 5mg: 15.0% body weight loss at 72 weeks
  • Tirzepatide 10mg: 19.5% body weight loss at 72 weeks
  • Tirzepatide 15mg: 20.9% body weight loss at 72 weeks
  • Placebo: 3.1% body weight loss

In a separate sub-group analysis, participants who entered the trial with obesity but without diabetes lost an average of 22.5% at the highest dose — significantly greater than semaglutide's 14.9%.

Cost in India: Prescription vs Research Grade

Prescription Mounjaro in India (where available through import):

  • 2.5mg pen: ₹18,000–22,000
  • 5mg pen: ₹20,000–25,000
  • Per month cost: ₹20,000–25,000 (one pen per week)

Research-grade tirzepatide from PeptideBazaar:

  • Tirzepatide 5mg vial: ₹4,100
  • Per month research cost: substantially lower

Dosing Titration Protocol

  • Weeks 1–4: 2.5mg once weekly
  • Weeks 5–8: 5mg once weekly
  • Weeks 9–12: 7.5mg once weekly (optional, based on tolerance)
  • Week 13+: 10–15mg once weekly (maximum research dose)

Subcutaneous injection, abdomen, thigh, or upper arm. Rotate sites weekly. Slow titration is essential — GI side effects are dose-dependent and resolve with proper titration.

Insulin Resistance Benefits

Beyond weight loss, tirzepatide produces remarkable improvements in metabolic markers: HbA1c reduction of 2.0–2.3%, significant improvements in fasting glucose, triglycerides, HDL cholesterol, and blood pressure. For research into metabolic syndrome and insulin resistance in India (extremely prevalent), these effects make tirzepatide highly relevant.

Diet Considerations

Because tirzepatide dramatically reduces appetite, protein intake often drops below optimal. Prioritise protein at every meal — target 1.6–2g/kg of body weight daily to preserve lean mass during rapid fat loss. Resistance training at least 3x weekly is strongly recommended to maintain muscle through the weight loss phase.
